I remember being about 19, reading a tiny paragraph in the paper about Hanford having a containment tank which was increasing in pressure and temperature, and none of the nuclear physicists could tell why. Apparently, no one kept records of -what- was poured into the tank, and no one knew what reactions were going on. The tank was too old and under such pressure they were afraid it would explode if they tried to transfer the materials.

Since then, every single thing I've read about the American nuclear program has only shown an equal apocalyptic inducing levels of irresponsibility.
